Depends on your plan. Call the number on the back of your insurance card and ask them. They’ll tell you.

What country are you in here in Aus, depending on your age and what the problem is, they are provided free through Australian Hearing. As are all your appointments. Of course if you want the best you still have to pay. What is the problem, and have you seen the audiologist.

No, most insurance companies do not pay for hearing aids.
Actually as of recently, a lot of states are introducing bills to mandate that they should have to.

At this time most insurance companies do not cover hearing aids and until Medicare decides to cover them (which they never will) insurance companies won’t either.
The exception is if you live in one of the 14 states that mandate hearing aid insurance coverage for children and/or dependents.

This will depend in the type of insurance or health plan you have, in buying an insurance you must make sure that you know what are the benefits they offer, it is better to carefully read the agreement and policies before signing anything
